Application for Transfer Students
To apply for admissions to St. Teresa's Academy, please complete and mail the Transfer Application Form to:

Also provide the following material to the Office of Admission:
1. Official copy of complete high school transcript
2. Three letters of recommendation from teachers, administrator or
counselors of your current school
Checklist:
- Print and complete the application form.
- Obtain a copy of your current official transcript.
- Obtain three letters of recommendation.
- Mail or deliver all of the above information to the Admissions Director.
- (Please note: Students with D’s or F’s on their transcript will not be considered)
