To apply for admissions to St. Teresa's Academy, please complete and mail the STA application form, transcript release form and Placement Test registration form to:

The Office of Admission of St. Teresa's Academy will request the following information from your current school:
1. Official copy of complete high school transcript.
2. Three letters of recommendation from teachers, administrator or
counselors of your current school.
